Opioids don’t relieve acute low back or neck pain – and can result in worse pain, new study finds

In Australia, around <a href="https://link.springer.com/article/10.1007/s00586-017-5178-4">40% of people</a> with low back and neck pain who present to their GP and <a href="https://qualitysafety.bmj.com/content/28/10/826">70% of people</a> with low back pain who visit a hospital emergency department are prescribed opioids such as oxycodone.</p> <p>But our <a href="https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(23)00404-X/fulltext">new study</a>, published today in the Lancet medical journal, found opioids do not relieve “acute” low back or neck pain (lasting up to 12 weeks) and can result in worse pain.</p> <p>Prescribing opioids for low back and neck pain can also cause <a href="https://www.healthdirect.gov.au/taking-opioid-medicines-safely">harms</a> ranging from common side effects – such as nausea, constipation and dizziness – to <a href="https://www.aihw.gov.au/reports/illicit-use-of-drugs/opioid-harm-in-australia/summary">misuse, dependency, poisoning and death</a>.</p> <p>Our findings show opioids should <em>not</em> be recommended for acute low back pain or neck pain. A change in prescribing for low back pain and neck pain is urgently needed in <a href="https://www.tga.gov.au/resources/publication/publications/addressing-prescription-opioid-use-and-misuse-australia">Australia</a> and <a href="https://www.thelancet.com/commissions/opioid-crisis">globally</a> to reduce opioid-related harms.</p> <h2>Comparing opioids to a placebo</h2> <p>In our trial, we randomly allocated 347 people with acute low back pain and neck pain to take either an opioid (oxycodone plus naloxone) or <a href="https://www.betterhealth.vic.gov.au/health/conditionsandtreatments/placebo-effect">placebo</a> (a tablet that looked the same but had no active ingredients).</p> <p>Oxycodone is an opioid pain medicine which can be given orally. <a href="https://www.nps.org.au/radar/articles/oxycodone-with-naloxone-controlled-release-tablets-targin-for-chronic-severe-pain">Naloxone</a>, an opioid-reversal drug, reduces the severity of constipation while not disrupting the pain relieving effects of oxycodone.</p> <p>Participants took the opioid or placebo for a maximum of six weeks.</p> <p>People in the both groups also received <a href="https://www.sciencedirect.com/science/article/pii/S1836955321000941">education and advice</a> from their treating doctor. This could be, for example, advice on returning to their normal activities and avoiding bed rest.</p> <p>We assessed their outcomes over a one-year period.</p> <h2>What did we find?</h2> <p>After six weeks of treatment, taking opioids did not result in better pain relief compared to the placebo.</p> <p>Nor were there benefits to other outcomes such as physical function, quality of life, recovery time or work absenteeism.</p> <p>More people in the group treated with opioids experienced nausea, constipation and dizziness than in the placebo group.</p> <p>Results at one year highlight the potential long-term harm of opioids even with short-term use. Compared to the placebo group, people in the opioid group experienced slightly worse pain, and reported a higher risk of <a href="https://academic.oup.com/painmedicine/article/20/1/113/4728236#129780622">opioid misuse</a> (problems with their thinking, mood or behaviour, or using opioids differently from how the medicines were prescribed).</p> <p>More people in the opioid group reported pain at one year: 66 people compared to 50 in the placebo group.</p> <h2>What will this mean for opioid prescribing?</h2> <p>In recent years, international low back pain guidelines have shifted the focus of treatment from drug to non-drug treatment due to <a href="https://www.thelancet.com/article/S0140-6736(18)30489-6/fulltext">evidence</a> of limited treatment benefits and concern of medication-related harm.</p> <p>For acute low back pain, <a href="https://link.springer.com/article/10.1007/s00586-018-5673-2">guidelines</a> recommend patient education and advice, and if required, anti-inflammatory pain medicines such as ibuprofen. Opioids are <a href="https://link.springer.com/article/10.1007/s00586-018-5673-2">recommended only</a> when other treatments haven’t worked or aren’t appropriate.</p> <p>Guidelines for <a href="https://pubmed.ncbi.nlm.nih.gov/33064878/">neck</a> pain similarly discourage the use of opioids.</p> <p>Our latest research clearly demonstrates the benefits of opioids do not outweigh possible harms in people with acute low back pain and neck pain.</p> <p>Instead of advising opioid use for these conditions in selected circumstances, opioids should be discouraged without qualification.</p> <h2>Change is possible</h2> <p>Complex problems such as opioid use need smart solutions, and another study we recently conducted provides convincing data opioid prescribing can be successfully reduced.</p> <p>The <a href="https://qualitysafety.bmj.com/content/30/10/825">study</a> involved four hospital emergency departments, 269 clinicians and 4,625 patients with low back pain. The intervention comprised of:</p> <ul> <li>clinician education about <a href="https://aci.health.nsw.gov.au/networks/musculoskeletal/resources/low-back-pain">evidence-based management</a> of low back pain</li> <li>patient education using posters and handouts to highlight the benefits and harms of opioids</li> <li>providing heat packs and anti-inflammatory pain medicines as alternative pain-management treatments</li> <li>fast-tracking referrals to outpatient clinics to avoid long waiting lists</li> <li>audits and feedback to clinicians on information about opioid prescribing rates.</li> </ul> <p>This intervention reduced opioid prescribing from <a href="https://qualitysafety.bmj.com/content/30/10/825">63% to 51% of low back pain presentations</a>. The <a href="https://emj.bmj.com/content/early/2023/04/02/emermed-2022-212874">reduction was sustained for 30 months</a>.</p> <p>Key to this successful approach is that we worked with clinicians to develop suitable pain-management treatments without opioids that were feasible in their setting.</p> <p>More work is needed to evaluate this and other interventions aimed at reducing opioid prescribing in other settings including GP clinics.</p> <p>A nuanced approach is often necessary to avoid causing <a href="https://theconversation.com/opioid-script-changes-mean-well-but-have-left-some-people-in-chronic-pain-156753">unintended consequences</a> in reducing opioid use.</p> <p>If people with low back pain or neck pain are using opioids, especially at higher doses over an extended period of time, it’s important they seek advice from their doctor or pharmacist before stopping these medicines to avoid <a href="https://www.healthdirect.gov.au/opioid-withdrawal-symptoms">unwanted effects when the medicines are abruptly stopped</a>.</p> <p>Our research provides compelling evidence opioids have a limited role in the management of acute low back and neck pain. The challenge is getting this new information to clinicians and the general public, and to implement this evidence into practice.<!-- Below is The Conversation's page counter tag. 8 ways to stretch your cruise budget

<p>When it comes to cruising there are plenty of ways you can make your budget stretch further without have to give up on the quality of holiday you want to have.</p> <p><strong>Be flexible</strong></p> <p>You can snag a great deal if you’re flexible with dates and open to new destinations. Fares can fluctuate on a daily basis so, if you aren’t locked in to a certain schedule, you could grab a great last minute bargain. Plus, you might discover an amazing new destination you’d never thought of.</p> <p><strong>Shop the sales</strong></p> <p>The cruise industry has sales on all year round and plenty of cruise lines, travel agents, forums and cruise websites will have deals pages or newsletters you can sign up to. If you stay abreast of all the special offers that are coming out, you can be sure to grab the best one.</p> <p><strong>Buy a package</strong></p> <p>Dining at the main restaurants will be included in your fare, but each ship will have a few specialty restaurants that come at an extra charge. If you want to try them all, buy a package that will come at a significant discount. Same goes for alcohol. If you think you’re likely to drink a few cocktails in the afternoon and wine with dinner, buy a daily drinks package to lower the cost.</p> <p><strong>Save on port days</strong></p> <p>Onboard spas are expensive, often even more expensive than you would find on shore. But most ships will have specials on offer during port days when most people are off the ship. If you don’t mind missing the destination you can get a great deal. The onboard shops will often have special sales or events on port days too. Check your daily cruise calendar.</p> <p><strong>Watch your tips</strong></p> <p>Almost all lines will add a daily gratuity to your account, usually around $12to 15 per person per day. You’ll also find an extra 15 to 18 per cent gratuity added to bar bills or spa treatments. That means you don’t have to tip any extra – even though they leave a space for it on the bill. You don’t want to end up tipping 30 per cent on a round of beers.</p> <p><strong>Disconnect at sea</strong></p> <p>Internet and phone calls are the two most expensive things on a cruise – which makes sense when you realise you’re in the middle of the ocean. Mobile roaming charges are steep and cruise ship wifi extortionate, so the best thing to do is unplug your devices. You can always find a free wifi hotspot or payphone when you’re on shore.</p> <p><strong>Guide yourself</strong></p> <p>Shore excursions are a great way to experience a port, but they can also be really expensive. And many of them aren’t worth the money, like beach days or city bus tours. At most places you can get a taxi to take you to the beach for a fraction of the price or find a local bus that will take you on the same route. Do a little internet research before you go and reap the rewards.</p> <p><strong>Use an agent</strong></p> <p>A lot of money saving tactics come from a little DIY, but using a specialist cruise travel agent can actually save you money in the long run. 6 stretches for lower back pain

<h2>Lower back pain 101</h2> <p>Low back pain is a beast. And not just because it can make you feel like your body is rebelling against you. Back pain can is a symptom of a problem. To treat the symptom – with things like stretches for lower back pain – you have to know what the problem is.</p> <p>This becomes complicated because the cause of low back pain is sometimes hard to pin down. Sometimes it’s obvious. You pick up a heavy box without thinking about proper lifting form and – bam! – you pull a muscle and end up with an acute bout of pain. But other times, the problem is more insidious and unclear.</p> <p>Maybe after years of inactivity, you’ve felt pain starting to creep in, possibly due to changes in muscle balance, decreases in strength, and general inflammation. Maybe you injured yourself years ago and, over time, a minor injury exacerbated it into something more serious.</p> <h2>Here’s why stretching could help lower back pain</h2> <p>That doesn’t mean you have to live with the pain. Movement (you know, exercise) is one of the best ways to prevent, limit, and address back pain.</p> <p>“Most of us aren’t moving in the variety of ways our bodies are built for, and habitual postures and sedentary living create excessive muscular tension, which leads to discomfort,” says Lara Heimann, a licensed physical therapist and yoga instructor. “Our bodies signal to us ‘please move!’ But we often ignore the signals and instead adapt to not listening. Over time, we lose some of the innate brain signallings that would otherwise keep us healthy and mobile.”</p> <p>The answer, then, is to move more. You need a well-rounded exercise program that includes back pain exercises, as well as cardiovascular and muscle-strengthening exercises. Incorporating flexibility and stretching exercises can also help you attain and maintain the range of motion you need to help keep pain at bay.</p> <p>“Taking time to stretch helps decrease stiffness, reduces pain and discomfort, and reprograms our brains to send signals when we need to get up and stretch,” Heimann says.</p> <p>Regular stretching can also facilitate better circulation, allowing for more efficient transportation of oxygen and nutrient-rich blood throughout the body, says Heimann.</p> <p>But most importantly, Heimann stresses that stretching is a preservation tool for movement, helping prevent tightness and tension that can set the stage for injuries. “Regular stretching helps to preserve joint health,” Heimann says. “The joint can move freely and efficiently in all directions with decreased stress placed on its structures.”</p> <h2>Word of caution</h2> <p>Back pain has so many causes and some are more serious than others. If your back pain persists for more than a few days, or if it’s severe or causes weakness or tingling in your extremities, make an appointment to see your doctor. Also, avoid any stretch or exercise that causes sharp or shooting pain. Not all types of stretches or exercises are appropriate for every person or every injury. This is why extended self-treatment isn’t advised. To address an issue effectively, you need to know exactly what the issue is.</p> <h2>Warm up</h2> <p>You know how you feel a little more tight first thing in the morning or after sitting still for a few hours? While stretching is a good way to “wake up” your muscles and joints after remaining stationary, you’ll feel better all-around if you’ve gotten your blood pumping a little bit first. And you’ll likely move more smoothly and comfortably through a full range of motion as you stretch.</p> <p>This is because circulating blood delivers oxygen and nutrients to your working muscles, helps warm them up, and prepares them for whatever movements you’re about to ask of them.</p> <p>Walking briskly or marching in place for a few minutes before you dive in is a good way to help prep your body for your stretching routine. For each of the exercises below, an exercise mat is recommended.</p> <h2>Cat-Cow</h2> <p>For those with back pain, cat-cow – an exercise that is popular in yoga – is a good, active stretch that takes the spine through flexion and extension. According to Heimann, it can help improve posture. Plus, it’s one of the best stretches to do for morning back pain relief.</p> <h3>How to do Cat-Cow</h3> <p>Start on your hands and knees on the floor in a “tabletop” position (back flat, knees under your hips, palms under your shoulders). In a controlled action, press your tailbone up and slowly extend your spine by allowing your abdomen to release toward the floor as you press your chest and shoulders up. You should create a U-shaped curve in your back as you look up toward the ceiling. Think about moving each vertebra separately in a wave-like fashion as you create the extension.</p> <p>Hold for a second, then reverse the action. This time, release your head and neck toward the floor as you flex your spine, ultimately tucking your tailbone under to create a rainbow-like arc with your spine.</p> <p>The first position is “cat,” and the second position is “cow.” As you move into cat, inhale; as you move into cow, exhale. Continue alternating between the two positions for a total of 30 to 60 seconds to help release tension in the back.</p> <h2>Prone Press-up</h2> <p>The Prone Press-up is sometimes called the cobra pose in yoga. But it’s also an essential part of the McKenzie Method that physical therapists and other clinicians often use to help address back pain. While people with low back pain might assume that back flexion (stretches like forward bends to touch the toes) should be used to help loosen up tight hamstrings, hips, and low back, the research actually suggests otherwise. Many people with low back pain are better served by doing back extension exercises, like the prone press-up.</p> <p>“Through clinical observation and research, we have seen that moving into the direction of lumbar spine extension can make a tremendous difference in many people with low back pain,” says chiropractor Jordan Duncan.</p> <p>“One reason for the great benefit of moving into spinal extension is likely due to the fact that as a society we spend the bulk of our time in flexion and very little time in extension,” he says. “Therefore, in the majority of people, spinal extension allows us to undo the effects of a great deal of time spent in flexion.”</p> <h3>How to do Prone Press-up</h3> <p>Lie on your stomach with your palms flat on the mat at your shoulders, as if you were going to do a pushup. Keeping your legs and hips on the floor, press through your palms and lift your chest from the mat, aiming to fully extend your elbows as you try to achieve full extension through your spine. Hold for one or two seconds, then slowly reverse the movement and lower your chest back to the floor.</p> <p>Continue repeating the exercise in a controlled fashion for 30 to 60 seconds. If you can’t reach full extension without pain or irritation, move to the range of motion you can comfortably achieve. As an alternative, press up to a partial extension balanced on your elbows.</p> <h2>Standing Backbend</h2> <p>If the Prone Press-up is simply not doable, the Standing Backbend is another option for achieving back extension. This can be particularly helpful if you experience localised pain while sitting, says physical therapist Sara Mikulsky. “When we spend long hours sitting, our core muscles can weaken and our hip flexors can tighten,” she says. “This then puts abnormal strain on the spine and its structure, which can lead to pain.”</p> <h3>How to do Standing Backbend</h3> <p>Stand tall with your feet roughly hip-distance apart and your knees very slightly bent. Place your hands on the back of your hips. Engage your core and gently lean back. Don’t overdo it, but do allow your shoulders to open and your chest to lift up toward the ceiling. Hold for a few seconds, then carefully return to the starting position. Repeat for 30 to 60 seconds.</p> <h2>Kneeling Hip Flexor</h2> <p>The Standing Backbend can help stretch the hip flexors while also releasing tension in the lumbar spine. You can also specifically target your hip flexors to help loosen them up with a Kneeling Hip Flexor.</p> <h3>How to do Kneeling Hip Flexor</h3> <p>Start on your knees, your hips extended and your torso tall. Place your right foot on the floor in front of you, with your right knee bent at a 90-degree angle (as if you’re about to propose). Place your hands on your hips and shift your weight forward without leaning your torso forward as you press and more fully extend your left hip flexor.</p> <p>You should feel the stretch at the front of your left hip. Hold for a second, release the stretch, then repeat. Continue for 30 to 60 seconds, then switch legs.</p> <h2>Supine Figure 4 Stretch</h2> <p>If you tend to have pain that travels down your leg, the best stretches might differ. Mikulsky points out that this form of pain may be caused by a nerve or disc injury that requires further medical attention. However, stretches – like the Supine Figure 4 stretch – that target a deep muscle in your buttocks called the piriformis, might provide some lower back pain relief (although they’re unlikely to fix the underlying problem).</p> <h3>How to do Supine Figure 4 Stretch</h3> <p>Lie on your back with your knees bent and your feet flat on the floor. Lift your right foot from the floor and place your ankle across your left thigh. Allow your right knee to open outward, creating a “4” shape with your legs. You should feel a stretch through the outside of your hip.</p> <p>Holding this position, reach your hands behind your left thigh and use your hands to guide your left knee closer to your chest. Lift your left foot from the floor to deepen the stretch through your right buttock and hip. Hold for a few seconds, then release your left foot to the floor. Repeat for 30 to 60 seconds before switching sides.</p> <h2>Knee to Chest</h2> <p>If you tend to have back pain while standing, Mikulsky says this might a joint issue or arthritis. If this is the case, a back extension might actually place more stress on your spine, leading to pain. (This is the tricky part about back issues – the stretches that are most appropriate for one issue might be the least appropriate for another.) If you find that standing or attempting back extension stretches – like the Standing Backbend or the Prone Press-up – cause more irritation or pain, try the Knee to Chest exercise.</p> <h3>How to do Knee to Chest</h3> <p>Lie on your back with your legs bent and feet on the floor. Place your hands on the back of each thigh and use your hands to guide your knees toward your chest. Place your hands on your shins and gently pull your knees even closer. You should feel the stretch across your lower back. The wild weather of La Niña could wipe out vast stretches of Australia’s beaches and sand dunes

<p>Australians along the east cost are bracing for yet another round of heavy rainfall this weekend, after a band of stormy weather soaked <a href="https://theconversation.com/on-our-wettest-days-stormclouds-can-dump-30-trillion-litres-of-water-across-australia-191949">most of the continent</a> this week.</p> <p>The Bureau of Meteorology has <a href="https://www.youtube.com/watch?v=QUsNQ_-fNbM&amp;ab_channel=BureauofMeteorology">alerted</a> southern inland Queensland, eastern New South Wales, Victoria and northern Tasmania to ongoing flood risks, as the rain falls on already flooded or saturated catchments.</p> <p>This widespread wet weather heralds <a href="https://theconversation.com/la-nina-3-years-in-a-row-a-climate-scientist-on-what-flood-weary-australians-can-expect-this-summer-190542">Australia’s rare third</a> back-to-back La Niña, which goes hand-in-hand with heavy rain. There is, however, another pressing issue arising from La Niña events: coastal erosion.</p> <p>The wild weather associated with La Niña will drive more erosion along Australia’s east coast – enough to wipe out entire stretches of beaches and dunes, if all factors align. So, it’s important we heed lessons from past storms and plan ahead, as climate change <a href="https://theconversation.com/2022s-supercharged-summer-of-climate-extremes-how-global-warming-and-la-nina-fueled-disasters-on-top-of-disasters-190546">will only exacerbate</a> future coastal disasters.</p> <figure><iframe src="https://www.youtube.com/embed/QUsNQ_-fNbM?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe><figcaption><span class="caption">Ongoing flood risk for eastern Australia | Bureau of Meteorology.</span></figcaption></figure> <h2>How La Niña batters coastlines</h2> <p>La Niña is associated with warmer waters in the western Pacific Ocean, which increase storminess off Australia’s east coast. Chances of a higher number of tropical cyclones increase, as do the chances of cyclones travelling further south and further inland, and of more frequent passages of east coast lows.</p> <p>Australians had a taste of this in 1967, when the Gold Coast was hit by the largest storm cluster on record, made up of four cyclones and three east coast lows within six months. 1967 wasn’t even an official La Niña year, with the index just below the La Niña threshold.</p> <p>Such frequency didn’t allow beaches to recover between storms, and the overall erosion was unprecedented. It <a href="https://impact.griffith.edu.au/seawall-engineering/">forced many</a> local residents to use anything on hand, even cars, to protect their properties and other infrastructure.</p> <p>Official La Niña events occurred soon after. This included a double-dip La Niña between 1970 and 1972, followed by a triple-dip La Niña between 1973 and 1976.</p> <p>These events fuelled two cyclones in 1972, two in 1974 and one in 1976, wreaking havoc along the entire east coast of Australia. Indeed, 1967 and 1974 are considered <a href="https://www.dailytelegraph.com.au/newslocal/northern-beaches/one-of-the-storms-that-hit-us-in-1974-was-among-the-three-worst-since-white-settlement/news-story/0cd5ca874d6b37206762d8485e4eb442">record years</a> for storm-induced coastal erosion.</p> <p>Studies show the extreme erosion of 1974 was caused by a combination of large waves coinciding with <a href="https://ro.uow.edu.au/cgi/viewcontent.cgi?article=1066&amp;context=scipapers">above-average high tides</a>. It took over ten years for the sand to come back to the beach and for <a href="https://www.jstor.org/stable/4300263">dunes to recover</a>. However, <a href="https://www.nature.com/articles/s43247-022-00437-2">recent studies</a> also show single extreme storms can bring back considerable amounts of sand from deeper waters.</p> <p>La Niña also modifies the direction of waves along the east coast, resulting in waves approaching from a more easterly direction (<a href="https://www.surfline.com/surf-news/forecaster-blog-la-nina-conditions-mean-surf/97904">anticlockwise</a>).</p> <p>This subtle change has huge implications when it comes to erosion of otherwise more sheltered <a href="https://au.news.yahoo.com/how-la-nina-may-damage-queensland-tourist-hot-spots-041805874.html">north-facing beaches</a>. We saw this during the recent, and relatively weaker, double La Niña of 2016-18.</p> <p>In 2016, an east coast low of only moderate intensity produced extreme erosion, similar to that of 1974. Scenes of destruction along NSW – including a collapsed backyard pool on <a href="https://www.wrl.unsw.edu.au/news/wrl-coastal-engineers-document-the-worst-erosion-at-collaroy-since-1974">Collaroy Beach</a> – are now <a href="https://www.abc.net.au/news/2016-06-06/nsw-weather-large-waves-hit-collaroy-coast/7479846#:%7E:text=NSW%20weather%3A%20Collaroy%20swimming%20pool%20collapses%20as%20giant%20waves%20hit%20beachfront%20houses,-Posted%20Sun%205&amp;text=Waves%20up%20to%208%20metres,as%20wild%20weather%20battered%20NSW.">iconic</a>.</p> <p><a href="https://www.nature.com/articles/s41598-017-05792-1">This is largely</a> because wave direction deviated from the average by 45 degrees anticlockwise, during winter solstice spring tides when water levels are higher.</p> <h2>All ducks aligned?</h2> <p>The current triple-dip La Niña started in 2020. Based on Australia’s limited record since 1900, we know the final events in such sequences tend to be the weakest.</p> <p>However, when it comes to coastal hazards, history tells us smaller but more frequent storms can cause as much or more erosion than one large event. This is mostly about the combination of storm direction, sequencing and high water levels.</p> <p>For example, Bribie Island in Queensland was hit by relatively large easterly waves from ex-Tropical Cyclone Seth earlier this year, coinciding with above-average high tides. This caused the island to <a href="https://www.abc.net.au/news/2022-01-27/bribie-island-changes-could-create-new-caloundra-bar/100777038#:%7E:text=Ex%2DTropical%20Cyclone%20Seth%20has,splitting%20the%20island%20in%20two.">split in two</a> and form a 300-metre wide passage of seawater.</p> <p>Further, the prolonged period of easterly waves since 2020 has already taken a toll on beaches and dunes in Australia.</p> <p>Traditionally, spring is the season when sand is transported onshore under fair-weather waves, building back wide beaches and tall dunes nearest to the sea. However, beaches haven’t had time to fully recover from the previous two years, which makes them more vulnerable to future erosion.</p> <p>Repeated <a href="https://www.usc.edu.au/about/structure/schools/school-of-science-technology-and-engineering/coast4d">elevation measurements</a> by our team and citizen scientists along beaches in the Sunshine Coast and Noosa show shorelines have eroded more than 10m landwards since the beginning of this year. As the photo below shows, 2-3m high erosion scarps (which look like small cliffs) have formed along dunes due to frequent heavy rainfalls and waves.</p> <p>On the other hand, we can also see that the wet weather has led to greater growth of vegetation on dunes, such as native spinifex and dune bean.</p> <p>Experiments in laboratory settings show dune vegetation can dissipate up to 40-50% of the water level reached as a result of waves, and <a href="https://www.sciencedirect.com/science/article/abs/pii/S0272771418307583">reduce erosion</a>. But whether this increase in dune vegetation mitigates further erosion remains to be seen.</p> <h2>A challenging future</h2> <p>The chances of witnessing coastal hazards similar to those in 1967 or 1974 in the coming season are real and, in the unfortunate case they materialise, we should be ready to act. Councils and communities need to prepare ahead and work together towards recovery if disaster strikes using, for example, sand nourishment and sandbags.</p> <p>Looking ahead, it remains essential to further our understanding about coastal dynamics – especially in a <a href="https://www.abc.net.au/news/2022-09-03/gold-coast-no-stranger-to-beach-erosion/101381812#:%7E:text=a%20huge%20challenge-,Millions%20spent%20to%20protect%20Gold%20Coast%20beaches%2C%20but,change%20poses%20a%20huge%20challenge&amp;text=In%201967%2C%20Gold%20Coast%20beaches,and%20ruined%20the%20tourist%20season.">changing climate</a> – so we can better manage densely populated coastal regions.</p> <p>After all, much of what we know about the dynamics of Australia’s east coast has been supported by coastal monitoring programs, which were implemented <a href="https://www.publications.qld.gov.au/dataset/coastal-observation-program-engineering">along Queensland</a> and NSW after the 1967 and 1974 storms.</p> <p>Scientists predict that La Niña conditions along the east coast of Australia – such as warmer waters, higher sea levels, stronger waves and more waves coming from the east – will become <a href="https://theconversation.com/climate-fuelled-wave-patterns-pose-an-erosion-risk-for-developing-countries-184064">the norm under climate change</a>.</p> <p>It’s crucial we start having a serious conversation about coastal adaptation strategies, including implementing a <a href="https://www.usc.edu.au/about/unisc-news/news-archive/2022/january/coastal-erosion-may-force-retreat-from-the-sea#:%7E:text=Giving%20up%20land%20to%20the,of%20the%20Sunshine%20Coast%20researcher.">managed retreat</a>. 17th-century Polish ‘vampire’ found buried with sickle across neck

<p dir="ltr">The remains of a woman found in a 17th-century graveyard in Poland are believed to be an example of an ‘anti-vampire’ burial after a sickle was also found placed across her neck to prevent her from rising from the dead.</p> <p dir="ltr">Dariusz Poliński, a professor at Nicholas Copernicus University, led the archaeological dig where the remains were uncovered, with the <em><a href="https://www.dailymail.co.uk/news/article-11173505/Remains-VAMPIRE-pinned-ground-sickle-throat-Poland.html" target="_blank" rel="noopener">Daily Mail</a></em> reporting that the skeleton was found wearing a silk cap and with a protruding front tooth.</p> <p dir="ltr">“The sickle was not laid flat but placed on the neck in such a way that if the deceased had tried to get up… the head would have been cut off,” Professor Poliński told the outlet.</p> <p dir="ltr">According to the <em><a href="https://www.smithsonianmag.com/smart-news/17th-century-poland-vampires-werent-boogeymen-out-town-girl-or-boy-next-door-180953476/" target="_blank" rel="noopener">Smithsonian</a></em> magazine, Eastern Europeans reported fears of vampires and began treating their dead with anti-vampire rituals during the 11th century.</p> <p><span id="docs-internal-guid-e1199576-7fff-cba7-8161-75f5e8ce3f2a">By the 17th century, these practices were common across Poland in response to reports of a vampire outbreak, per <em><a href="https://www.sciencealert.com/research-reveals-the-origin-of-poland-s-mysterious-vampires" target="_blank" rel="noopener">ScienceAlert</a></em>.</span></p> <p><img src="https://oversixtydev.blob.core.windows.net/media/2022/09/skeleton-lady1.jpg" alt="" width="1280" height="720" /></p> <p dir="ltr"><em>The skeletal remains, pictured from above. Image: Łukasz Czyżewski, NCU</em></p> <p dir="ltr">Professor Poliński told the New York Post that there were other forms of protection to prevent vampires from returning from the dead, including cutting off limbs and using fire.</p> <p dir="ltr">“Other ways to protect against the return of the dead include cutting off the head or legs, placing the deceased face down to bite into the ground, burning them, and smashing them with a stone,” he said.</p> <p dir="ltr">The skeleton’s toe was also padlocked, which Professor Poliński said likely symbolised “the closing of a stage and the impossibility of returning”.</p> <p dir="ltr">This isn’t the first time a ‘vampire’ has been discovered by archaeologists either.</p> <p dir="ltr">Matteo Borrini, a lecturer at Liverpool John Moore University, discovered the remains of a woman who died in the 16th century and was buried with a stone in her mouth in a mass grave with plague victims.</p> <p dir="ltr">He explained that outbreaks of ‘vampires’ were often associated with periods where people were dying from unknown causes at the time - such as pandemics or mass poisoning.</p> <p dir="ltr">“These ‘vampires’ start to hunt and kill family members first, then the neighbours, and then all the other villages,” he said.</p> <p dir="ltr">“This is the classical pattern of a disease that is contagious.”</p> <p dir="ltr">The most recent ‘vampire’ remains, which were dug up in August, are being further investigated by scientists.</p> <p><span id="docs-internal-guid-c133c871-7fff-ea85-c6ce-820b4d4d2ba2"></span></p> <p dir="ltr"><em>Image: Mirosław Blicharski</em></p>

Woman avoids baggage fees with genius neck pillow hack

<p><span style="font-weight: 400;">While many travellers love to take advantage of cheap airline seats, the deals often come hand in hand with hefty baggage fees.</span></p> <p><span style="font-weight: 400;">One savvy TikTok user has devised a unique way to avoid the extra charges for luggage, using a travel neck pillow. </span></p> <p><span style="font-weight: 400;">The traveller, who shares videos under the name @anayotothe on TikTok, uses the cheap neck pillow as a secret carry-on bag to get out of those pesky extra fees. </span></p> <p><span style="font-weight: 400;">"Trying the Spirit and Frontier pillow hack cuz I ain't tryna pay $60 for a carry on," the woman wrote on the video, referencing two American airlines.</span></p> <p><span style="font-weight: 400;">She captioned the video, “My flight is in three hours let’s see how this goes!”</span></p> <p><span style="font-weight: 400;">The woman picked up the travel pillow for just $9 from the US supermarket Walgreens, and proceeded to take the pillow stuffing out of the case. </span></p> <p><span style="font-weight: 400;">She then stuffed all the clothes that wouldn’t fit in her backpack into the pillowcase, which could still be used to stay comfortable during her flight. </span></p> <p><span style="font-weight: 400;">"Everything fits except for this shirt!" the woman exclaimed, while showing followers her clever technique.</span></p> <p><span style="font-weight: 400;">After sharing the video with her followers, the woman later confirmed that she boarded her flight successfully with her hidden carry-on luggage.</span></p> <p><span style="font-weight: 400;">"On the way to Vegas and the way back, I didn't have to pay, with my travel pillow and my backpack," she said.</span></p> <p><span style="font-weight: 400;">While dozens of people commented to thank the savvy traveller for the tip, others also questioned if it was worth it to save money. </span></p> <p><span style="font-weight: 400;">When questioned about her methods, the woman responded, "It took five minutes, also it was an hour flight… there was no reason I should pay $300 for a flight if you can just take one of the budget airlines."</span></p> <p><em><span style="font-weight: 400;">Image credits: Getty Images</span></em></p>

Wild elk finally gets tyre removed from around its neck after two years

<p dir="ltr">A wild elk in Colorado is free after a years-long ordeal to remove a rubber tyre from around its neck. Wildlife officers were<span> </span><a rel="noopener" href="https://cpw.state.co.us/aboutus/Pages/News-Release-Details.aspx?NewsID=7971" target="_blank">able to free it</a><span> </span>over the weekend after local residents alerted them to the creature’s location.</p> <p dir="ltr">The elk was a four-and-a-half year-old male weighing over 600 pounds, or over 270 kilograms. He had spent the past few years travelling between neighbouring counties, disappearing for long periods of time, particularly in the winter, and acting normally for a wild animal, not wanting to be around humans.</p> <blockquote class="twitter-tweet"> <p dir="ltr">The saga of the bull elk with a tire around its neck is over. While conducting a population survey for bighorn sheep and mountain goats, an officer saw the bull through a spotting scope. Wildlife officer Scott Murdoch said, “Being up in the wilderness, we didn’t really expect to be able to get our hands on the elk just because of the proximity or the distance away from civilization.</p> <p dir="ltr">“It is harder to get the further they are back in there and usually the further these elk are away from people, the wilder they act. That certainly played true the last couple of years, this elk was difficult to find, and harder to get close to.”</p> <p dir="ltr">In the past week alone, officers had made four attempts to try and tranquilise the animal. The potato trick to help get rid of neck pain and migraines

<p>An expert has revealed an unlikely item that serves to relieve neck pain and migraines.</p> <p>Those dealing with neck stiffness could use humble potatoes to treat themselves, said Stephen Makinde, the clinical director of Perfect Balance Clinic in London.</p> <p>“We’ve seen an increase in the number of clients with neck problems and migraines associated with neck stiffness since the start of the coronavirus lockdown,” Makinde told the <em><a href="https://www.dailystar.co.uk/health/bizarre-baking-potato-trick-could-22096583">Daily Star</a></em>.</p> <p>The problem could be attributed to increased use of electronic devices during the pandemic, which affects the upper back, he said.</p> <p>“If you are looking down, which you do when working on a laptop, what tends to happen is the head starts to hang forward and that puts a lot of strain on the upper back,” he said.</p> <p>“This can often cause a burning sensation in the trapezius muscles and affects other muscles in the neck, the spine and the nerves.”</p> <p>Makinde said lying down with tennis balls placed underneath the back of the head can help loosen muscles, release tension and help “reset the neck position”.</p> <p>“Tennis balls are really useful for this, but most people don’t have tennis balls at home,” he said.</p> <p>“So baking potatoes work well, too. How plant-based meat is stretching New Zealand’s cultural and legal boundaries

<p>Earlier this year, the New Zealand-based pizza chain <a href="https://www.stuff.co.nz/life-style/food-wine/78790234/the-history-of-hell-pizza">Hell Pizza</a> offered a limited-edition “Burger Pizza”. Its customers weren’t told that the “meat” was plant-based.</p> <p>Some customers <a href="https://www.stuff.co.nz/business/113824494/hell-pizza-covertly-dishes-up-beyond-meat-burger-patties">complained</a> to the Commerce Commission, which enforces consumer law in New Zealand. Yet, <a href="https://www.stuff.co.nz/business/113867599/scorned-hell-pizza-customers-bitter-over-fake-burger-meat">others</a> did not mind – or even appreciated – the move. The Commerce Commission, however, warned that the stunt likely breached consumer protection law.</p> <p>Hell Pizza’s ruse should catalyse discussion around the scope and purpose of consumer law, the culture of meat consumption and the future of animal farming. Under current law, “teaching through deception” is not possible. But we argue that consumer law needs to adopt a more nuanced approach.</p> <p><strong>Traditional legal approach</strong></p> <p>In October, the Commerce Commission <a href="https://comcom.govt.nz/case-register/case-register-entries/the-depths-lp-ta-hell-pizza/media-releases/commission-warns-hell-pizza-over-burger-pizza">warned</a> the pizza chain that it had probably breached the <a href="http://www.legislation.govt.nz/act/public/1986/0121/latest/DLM96439.html">Fair Trading Act 1986</a>. In particular, it had likely made false or misleading representations.</p> <p>The Commerce Commission <a href="https://comcom.govt.nz/__data/assets/pdf_file/0034/178792/Warning-letter-to-The-Depths-LP-trading-as-Hell-Pizza-Redacted-25-September-2019.pdf">stated</a> that a “burger traditionally includes a patty of minced beef” and “medium-rare is a term associated with meat, usually beef”.</p> <p>As a result, the pizza chain advised it had <a href="https://comcom.govt.nz/__data/assets/pdf_file/0034/178792/Warning-letter-to-The-Depths-LP-trading-as-Hell-Pizza-Redacted-25-September-2019.pdf">no intention</a> of engaging in this kind of campaign again. Interestingly, the pizza company has recently announced that the <a href="https://www.youtube.com/watch?v=TbaEo19Oc9k">Burger Pizza is back on the menu</a>.</p> <p>Australia’s consumer law around misleading and deceptive conduct is notably similar to New Zealand’s. In Australia, <a href="https://www.abc.net.au/news/2019-09-15/push-to-ban-milk-meat-seafood-labels-on-plant-based-produce/11513754">debates</a> around the meaning of the terms “milk”, “seafood” and “meat” are taking place. These discussions present an opportunity to rethink some of our conventions.</p> <p><strong>When is meat meat?</strong></p> <p>The traditional need to protect consumers from deceptive practices is clear. That said, it is perhaps also time to nudge consumers to reconsider their preconceptions and consumption of meat.</p> <p>Hell Pizza said it launched its plant-based meat product out of concerns for the future of the planet. According to the company, <a href="https://hellpizza.com/wickedpedia/2019/07/03/burger-pizza-statement">80% of consumers did not have an issue with being duped</a>, and 70% would order the pizza again.</p> <p>There are a few good reasons to reduce the amount of meat we eat. <a href="https://www.ncbi.nlm.nih.gov/pubmed/19339402">Research shows</a> that meat consumption is putting pressure on the environment. The amount of food and water required to raise animals for consumption <a href="https://news.cornell.edu/stories/1997/08/us-could-feed-800-million-people-grain-livestock-eat">exceeds</a> the nutrient value humans get from consuming meat. Further, livestock create <a href="https://ehp.niehs.nih.gov/doi/full/10.1289/ehp.11034">waste and emissions</a> that contribute to climate change.</p> <p>Plant-based meat may be more environmentally friendly. It also eliminates concerns around animal rights. Additionally, it is often perceived as a <a href="https://pdfs.semanticscholar.org/cf90/d287aa226b483aed430ff4f0432081bfd3d7.pdf">healthier alternative</a>.</p> <p><strong>Future foods</strong></p> <p>The plant-based meat industry faces two immediate challenges. The first is taste. If meat substitutes do not taste as good as animal-based meat, people will be <a href="http://freakonomics.com/podcast/meat/">less willing to consume them</a>.</p> <p>The second main challenge is cost. If plant-based meat is significantly more expensive than animal-based meat, consumers may opt for the latter.</p> <p>The cost of plant-based meat has become affordable enough for prominent market players, such as <a href="https://www.stuff.co.nz/business/116767086/dominos-adds-plantbased-meat-to-its-pizza-menu">Dominoes Pizza</a> and <a href="https://www.stuff.co.nz/business/farming/116657991/burger-king-finds-recipe-for-success-with-its-impossible-whopper">Burger King</a>, to offer plant-based products.</p> <p>Hell Pizza was not the first New Zealand company to offer its consumers plant-based meat products. In another controversy, Air New Zealand offered plant-based burgers in the business cabin on selected flights. This led to some criticism, including the deputy prime minister, Winston Peters, who was acting prime minister at the time, complaining that it was a “<a href="https://www.stuff.co.nz/business/farming/105216779/air-nzs-impossible-burger-criticised-by-former-primary-industries-minister">bad look</a>” for the airline not to promote New Zealand meat.</p> <p>Such a response is short-sighted. Animal farming is an important industry in New Zealand, <a href="http://www.environmentguide.org.nz/activities/agriculture/">contributing significantly</a> to the economy and social fabric. Because of its importance, New Zealanders should take seriously the potential impact of plant-based meat and the consequences of this emerging market.</p> <p><strong>Market disruption</strong></p> <p>Some companies have already stated their aspiration to completely <a href="http://freakonomics.com/podcast/meat/">replace animals as a food production technology</a> by 2035. The meat industry is likely to use its power to protect its interests. But these interests are not the only ones that should be voiced and considered.</p> <p>Instead of merely criticising companies that offer meat alternatives and use innovative marketing tools to do so, we should embrace these initiatives as an opportunity to rethink some of our conventions. We need to adapt to new realities in ways that make our societies more ethical, while also encouraging consumers to be more mindful of the environment and health-related aspects of their foods.</p> <p>The boundaries of consumer law should reflect this. The law regulates against misleading and deceptive conduct mainly because it is purportedly bad for consumers. However, the law should adopt a more holistic approach - one that considers the motivation for the allegedly misleading behaviour.</p> <p>Protecting consumers from deceptive conduct is not an end in itself. Perhaps the degree and context of the misleading behaviour should be considered against other legitimate objectives. Why do I have stretch marks and what can I do about them?

<p>Most women get them. Some men get them. Few people welcome them. Stretch marks, or <em>stria distensae</em> as they are known medically, are scars that appear when the skin is stretched beyond its <a href="http://physicsnet.co.uk/a-level-physics-as-a2/materials/hookes-law/">elastic limit</a>.</p> <p>Physicists define the elastic limit as the maximum force that can be applied to solid material before the onset of permanent deformation. In dermatology, when stresses up to the elastic limit are removed, the skin resumes its original size and shape. When forces beyond the elastic limit are removed, the skin remains permanently stretch-marked.</p> <p>The younger you are, the firmer your skin. The firmer your skin, the lower your elastic limit and the more likely you are to develop stretch marks. Stretch marks occur most frequently during adolescent or pregnancy growth.</p> <p>The primary cause is <a href="http://sro.sussex.ac.uk/41348/1/striae.pdf">mechanical</a> stretching of the skin due to underlying tissue expansion. Parallel inflammatory streaks appear and align perpendicular to the direction of skin tension. Microscopically, the skin is initially swollen, inflamed and elastin bundles in the inner layer of skin (the dermis) are disrupted.</p> <p>Over time, the inflammation eventually fades and is replaced by scar tissue. This produces a thinned outer layer of skin (the epidermis), loss of dermal elastin, and a replacement of the dermis by abnormally dense collagen fibres.</p> <p><strong>Risk factors</strong></p> <p>Adolescent stretch marks may appear on the lower abdomen, lower part of the back, buttocks, thighs and female breasts. They are most common on the thighs of girls and on the knees of boys.</p> <p>Hormonal and genetic factors are also involved in the development of pregnancy stretch marks. Girls who develop adolescent stretch marks on their breasts are more likely to develop abdominal stretch marks during pregnancy. Younger women, women who gain more weight during pregnancy, women with twins or large babies and women who go post-term are all <a href="http://www.ncbi.nlm.nih.gov/pmc/articles/PMC1913631/">more likely</a> to get stretch marks.</p> <p>Weightlifters are more susceptible to stretch marks, especially those who use anabolic <a href="http://example.com/http://www.betterhealth.vic.gov.au/bhcv2/bhcarticles.nsf/pages/Steroids">steroids</a>. Stretch marks can also occur in <a href="http://www.betterhealth.vic.gov.au/bhcv2/bhcarticles.nsf/pages/Cushing's_syndrome">Cushing’s syndrome</a> or following the administration of oral and topical <a href="http://www.betterhealth.vic.gov.au/bhcv2/bhcarticles.nsf/pages/Hormones_-_cortisol_and_corticosteroids">corticosteroids</a>.</p> <p><strong>Prevention and treatment</strong></p> <p>Creams and lotions can’t prevent stretch marks. While not all preventative treatments have been evaluated, and some treatments have shown promise in individual studies, a <a href="http://onlinelibrary.wiley.com/doi/10.1002/14651858.CD000066.pub2/abstract#leftBorder">Cochrane review</a> in 2012 concluded there is:</p> <blockquote> <p>no high-quality evidence to support the use of any of the topical preparations in the prevention of stretch marks during pregnancy.</p> </blockquote> <p>People with stretch marks have four options:</p> <p><strong>1) Learn to love them</strong>: Embrace stretch marks. Show them off. Post them on <a href="http://example.com/http://news.yahoo.com/viral-social-media-campaign-promotes-stretch-mark-acceptance-160825898.html">social media</a>. Wear them as a badge of honour.</p> <p><strong>2) Wait for them to fade</strong>: Most stretch marks begin red (stria rubra) and become white (stria alba) and less conspicuous over the course of a year or two. For people concerned by their stretch marks, reassurance is usually all that’s required.</p> <p><strong>3) Use creams to fade them</strong>: While creams won’t prevent stretch marks, they may help fade them, according to a recent comprehensive <a href="http://onlinelibrary.wiley.com/doi/10.1111/bjd.12681/abstract?deniedAccessCustomisedMessage=&amp;userIsAuthenticated=false">review</a>.</p> <p>Topical retinoid creams such as <a href="http://www.mayoclinic.org/drugs-supplements/tretinoin-topical-route/description/drg-20066521">tretinoin</a> are thought to work through induction of collagen synthesis and should be applied once daily for six months. Tretinoin works better on early red stretch marks, while white stretch marks respond poorly.</p> <p>Skin irritation is a common side effect of tretinoin, so it may not be suitable for people with sensitive skin. Cocoa butter is less irritating, but also less effective. Newer silicone gels seem to be more effective.</p> <p><strong>4) Laser</strong>: While pulsed dye vascular lasers or <a href="http://www.scfa.edu.au/skin-conditions/laser-treatments/intense-pulsed-light-ipl">intense pulse light</a> (IPL) treatments can fade red stretch marks, this is unnecessary, as most will fade naturally within six to 12 months.</p> <p>After a number of false starts, lasers have finally come of age for white stretch marks. The <a href="http://www.accessdata.fda.gov/cdrh_docs/pdf11/k110907.pdf">fractionated laser</a> works by burning tiny pin-prick-sized holes in the skin. The wounds are not visible to the naked eye, but microscopic wound healing to repair these tiny holes promotes new collagen formation and improves the skin thickness and appearance of the stretch marks.</p> <p>Responses vary but most people require four treatments, spaced four to six weeks apart, to achieve on average a 50 per cent improvement. Inflammation is common after treatment and generally lasts 24 to 48 hours.</p> <p>But for the vast majority of stretch marks, active treatment is not necessary. Most stretch marks naturally fade and become less conspicuous over time.<!-- Below is The Conversation's page counter tag. Research has revealed that technology is a literal pain in the neck

<p><span style="font-weight: 400;">Researchers have revealed that technology is changing the bones in our necks and skulls, which is setting people up for a lifetime of pain.</span></p> <p><span style="font-weight: 400;">This comes from device overuse, as it can cause posture problems.</span></p> <p><span style="font-weight: 400;">Researchers at the University of the Sunshine Coast have found something unusual that tends to go unnoticed.</span></p> <p><span style="font-weight: 400;">They used a sample of more than 1,200 people aged between 18 and 30. They discovered that 41 percent of the participants had grown a bony lump on the back of their skull.</span></p> <p><span style="font-weight: 400;">The horn-like spurs were between 10 to 30 millimetres thick.</span></p> <p><span style="font-weight: 400;">“We suspect the reason for this bone spur formation is because they carry their head forward,” University of the Sunshine Coast’s David Shahar said to </span><a href="https://thewest.com.au/news/health/researchers-reveal-technology-is-proving-a-pain-in-the-neck-ng-b881234805z"><span style="font-weight: 400;">The West.</span></a></p> <p><span style="font-weight: 400;">The sustained stress on the skull is creating bone de-position which usually takes years.</span></p> <p><span style="font-weight: 400;">The simple solution? Lift your head.</span></p> <p><span style="font-weight: 400;">“Bringing the weight of the head back to rest on the bones and not on the muscles,” Dr Shahar said. </span></p>

“Wild” and "incredible" weather set to hit: Urgent warning stretching 1000km

<p>The Bureau of Meteorology has issued a severe weather warning stretching 1000km as intense wind and rain are predicted to hit parts of the country.</p> <p>Catastrophic winds of more than 90km/h are set to hit certain areas – stretching from the north of Kempsey in NSW to Eden in the South Coast.</p> <p>Rob Sharpe, meteorologist for <em>Sky News Weather,</em> has said that most of NSW can expect to be drenched with rain on Tuesday, right around peak hour, making for “tricky” road conditions.</p> <p>“People will very much notice that change coming through,” he said.</p> <p>“It’s really going to bring some powerful winds. Australian cricket legend Matthew Hayden rushed to hospital after freak accident
